It involves safe and predictable medical procedures. Our clinic uses only renowned brands of implants – currently, the newest implants from Astra Tech – Astra Tech Implant System EV. An implant is an artificial root which can be inserted in place of one or more missing teeth. An implant is best placed as soon as possible after the loss of the tooth, but also possible are IMMEDIATE IMPLANTS, secured on the day of the tooth extraction. The implant is composed of titanium – a material which is capable of osseointegration, i.e., fusing with bone tissue. The implant has an internal thread on which we can fasten the connector, creating a foundation for the crown. The procedure of placing one implant lasts approx 30 minutes and can be performed in sedation, with the administration of analgesics, sedatives, and anti-oedematous drugs. An anaesthesiologist present during the procedure can also monitor the status of patients who suffer from general ailments, such as hypertension, heart disease, respiratory diseases, etc. Usually, however, sufficient is a local anaesthetic.

Implant osseointegration lasts between 2 and 6 months. After this time we can continue with preparing the prosthetic crown.

Implants are the best way to supplement missing teeth, as there is no need for sanding down the neighbouring teeth, implants function just like natural teeth, allow for very natural-looking crowns, and eliminate the need for using removable dentures, which are very inconvenient for patients.

Indications for implants include one or more missing teeth or the intolerance or poor stability of dentures. Inserting only two implants can already greatly improve the stability of dentures.

Contraindications for implants include:
– incomplete growth of the jaw bones
– patients under the age of 18
– severe general ailments
– endocarditis, prosthetic heart valves, organ transplants, rheumatism
– taking immunosuppressive, antidepressant, anticoagulant medication
– some psychiatric disorders

Other contraindications:
– poor hygiene
– undiagnosed craniofacial pain

– dysfunctions which can excessively burden the implant
– fibroma
– inappropriate bone structure

POST IMPLANT SURGERY RECOMMENDATIONS:
patients should:

– refrain from eating and drinking for 2-3 hours after the procedure, especially avoiding hot meals, as well as maintaining a soft diet for a few days
– avoid intensive physical activity for one week after the procedure, leading an energy-limiting lifestyle
– not smoke or drink alcohol until the completion of the antibiotic treatment
– use a new, soft, post-surgical toothbrush, use a recommended post-surgical mouthwash – e.g. Eludril, but rinse in a delicate manner especially over the first 2-3 days
– not overuse analgesic medication, such as Ibuprofen, as these could cause excessive bleeding. The recommended dose is 500 mg 3 times a day, and if necessary, additionally, Paracetamol 3 times a day
– contact their doctor in case of any worrying symptoms.

Properly used can achieve the finest results in the form of healthy, strong teeth and a beautiful, full smile. The main goal of prevention is to fight plaque – the cause of the development of caries and periodontitis. Piękna-Dent provides patients with a full range of preventive treatments performed professionally using modern yet proven methods.

Prevention of dental and periodontal diseases:
• Fluoride Varnish to prevent tooth decay.
• Removing tartar using an ultrasonic scaler and sanding of the teeth.

The most popular form of professional fluoride prevention is polishing the treated teeth using fluoride varnish such as Fluor Protector or Duraphat – recommended especially for children. It is widely used both in prevention of all teeth as well as selected areas. Coatings are characterized by a large viscosity, which provides their good adhesion to the tooth enamel, longer retention on the surface and slow release of fluoride. With these characteristics, these preparations provide decay reduction of up to 40%. The treatment should be repeated 2-4 times a year depending on the patient’s susceptibility to caries.

Tartar is the most serious cause of periodontal disease. During the therapy, it is very important to remove plaque (so-called scaling), thus inhibiting the disease in most cases.

According to all available research, scaling should be performed prophylactically on patients who do not have problems with periodontal disease, to prevent them.. The effect of this activity is practical elimination of paradontosis hazards from tartar deposition. Patients who had tartar fully removed at least once understand that this short and painless procedure helps to greatly reduce tooth decay and is critical for healthy gums. After professional cleaning, laying fluoride polish is recommended, which reduces tooth sensitivity and the risk of caries.

Sandblasting is used in order to thoroughly remove dental deposits, resulting from drinking coffee, tea, smoking cigarettes, which are difficult to remove by other means.

Sandblasting is the most common addition to teeth scaling procedure (the removal of tartar). However, it may also be a separate procedure, if there is no tartar on the teeth but the sediment only.
After removing the deposit the teeth regain their former lighter color and shine. Special powder ejected at high pressure enables special ‘sand’ to easily reach every part of the surface of the tooth, thus ensuring excellent cleaning effect.

The condition concerns mostly young people. The prevalence of bruxism is estimated at 8-20% of the population. It typically occurs during sleep, but may also occur during the day. Many people are unconscious of their teeth grinding habit. These individuals may not even be aware of these problems occurring during sleep and often wake up in the morning with sore jaw muscles or a headache. The forces activated on the chewing surfaces of patients with bruxism can be up to 10 times greater than those occurig in the normal process of biting and chewing. Bruxism can cause tooth pain, teeth grinding (eating), breaking tooth crowns, tooth loss, and may lead to pathological changes in the temporomandibular joint in the form of clicking and poping. The appearance of facial muscles also changes due to hypertrophy of masseter and temporal muscules, which makes the face look ‘square’.

The causes of bruxism may be different, both local and systemic. Stress is a dominant factor among the systemic causes of bruxism, therefore, it often concerns people in leadership positions and is an expression of experienced stress. Bruxism can be also caused by local factors such as chewing gum or chewing on one side. Other causes include: poorly shaped tooth filling, badly shaped surfaces of crowns, bridges or dentures and missing teeth. Symptoms of bruxism can be difficult to notice for a long time. Usually when patients suffer form different types of pain, sleep disturbances, poorer daytime function and chronic fatigue they will seek advice from the doctors of other specialties, usually a neurologist, laryngologist, psychiatrist or an ophthalmologist. Eventually, however, they will find their way to the dentist.

Pain caused by bruxism is often dull, deep and radiating. Headaches, shoulder girdle and arm aches as well as paresthesias may suggest neurological problems. Temple aches may suggest sinuses disease, and pain in the partoid area – ear diseases. Often a group of teeth ache and the patient is unable to tell which tooth specifically is causing the pain. Sometimes the patient says all his teeth hurt.

There are many treatments of bruxism ranging from psychotherapy to surgery. The appropriate treatment of bruxism depends on what is causing the problem. Based on several years of experience bruxism is currently treated using relaxation rails (transparent overlays for teeth) and btx (simultaneously). The overlay (relaxation rail) is made by a dentist to exactly match the teeth. It is slid on the teeth overnight and protects them from friction. Although effective in preventing the consequences of bruxism it does not cure the cause of the problem.

Btx is injected into the hypertrophic masseter and temporal muscles in the correct dose. The procedure is minimally invasive and not painful. Three days after the administration, the patient is relieved, muscle tension and pain are reduced. The treatment is repeated every 6 months. Based on 6 years of experience of experts in the field, after 2 years of treatment (the use of relaxing overlays and btx simultaneously) further treatment may be stopped. Usually after such period, the habit of teeth grinding disappears and gnashing muscles are reduced. The face ceases to look ‘square’ and it becomes leaner, rejuvenated and simply prettier.

The improvement of the appearance of teeth depend on many aspects. We must also be aware that missing teeth have a particularly negative effect on the general appearance by deepening the nasolabial furrows and reducing the lower part of the face which consequently give the impression of a prematurely aging face. In this regard, cosmetic dentistry is combined with aesthetic medicine and we must remember that before applying filler or lip augmentation we should fill in missing teeth.

Before using any treatments in the field of cosmetic dentistry we perform an x-ray diagnostics – an image covering all teeth (pantomogram). Based on photos and a detailed examination of the patient we set a treatment plan. During the same visit we should also professionally remove tartar. This procedure is performed using an ultrasonic scaler and a sandblaster and then polishing with polishing paste. It is the first step to get rid of the superficial sediments from coffee, tea and cigarettes. The subsequent professional hygiene instruction allows you to keep cleaning results for a long time.
The expectations concerning the procedure are based on individual conversations with a patient. If objections apply only to the colour of teeth, the whitening procedure can be carried out. There are two basic methods of whitening – in-office whitening using a Beyond lamp and overlay night whitening. Beyond whitening accelerator working together with special whitening kits ensures that the stains on the teeth are immediately oxidized. Specially designed gel is activated by a high-power light.

Whitening treatment is comfortable, safe, and on average the colour is lightened by 8 shades. The whitening agent oxidizes the entire surface of sixteen teeth in only one treatment, restoring them to a beautiful white. The treatment is recommended for people who have little time and want quick results. The overlay night whitening also gives good results. The patient is instructed to put on specially made caps with the whitening gel for 3-4 subsequent nights. Both methods will work best provided that for several days after the procedure we use only white diet – give up smoking!

Sometimes the patient’s concerns relate to the colour of one dead tooth – we can then apply the Opalescence Endo whitening product or make a prosthetic crown. If the concerns relate to the shape and position of teeth of young patients who wish to avoid invasive treatments we can make an adjustment in shape or position of teeth with composite material (we use excellent Gradia and Enamel materials). In this way you can, for example, close the gaps between the teeth. Making porcelain veneers is more invasive but also gives excellent cosmetic results. If the damage is more extensive and there are tooth gaps we can make all-ceramic crowns and bridges (see prosthetics).

The completion of missing teeth is very important for the patient, both for aesthetic and functional reasons (missing teeth can cause many different symptoms including pain in the temporomandibular joints, headaches and digestive disorders).

Fixed prosthetic devices (crowns and bridges) are usually recommended to the patients with missing teeth due to the convenience of reconstruction. With the loss of its root it is possible to restore a missing tooth with an implant (which is like an artificial root, located in the bone – see implantology). There are different types of materials from which we can make a crown but in our clinic we recommend zirconium crowns – excellent both for functional and aesthetical reasons.

The zirconia crown is made of zirconium oxide also known as “white gold” because of its excellent properties and resistance to bending in the range of 900MPa. For this reason, it is an alternative to crowns and bridges made of metal.

Zirconium oxide, in addition to its high compressive strength, is also characterized by a high light transmittance so the prosthetic material retains the optical properties of natural teeth. Prosthetic devices made on the zirconia substructures are characterized by high biocompatibility and give no allergic symptoms. The zirconia crown is created during the processes of CAD/CAM, CAD – (Computer Aided Design) and CAM (Computer Aided Manufacturing). The computer-designed crown (3D model) is cut from the block of material using a computer-controlled machining tools. In addition to the specific software and computers, the CAD/CAM system also consists of a scanner and a milling machine.

To sum up, the zirconium crown is the most technologically advanced achievement of modern dentistry. It is perfectly hermetic and our excellent lab will make it a unique work of art created just for you.

In addition to zirconium crowns we also recommend porcelain veneers, which can instantly improve the colour and shape of your teeth in a minimally invasive way. Of course, there are cases when we can not use fixed prosthetic devices and even implants due to some large bone defects.

In these cases we make removable dentures. To get the perfect sustenance of these dentures in the mouth we can use implants. It is an excellent solution in case of endentulous. It provides superior sustenance and support for both the denture and the soft tissue. Such dentures make furrows shallower and a face looks younger.
